Description：#衛部醫器製字第007625號 慢性腎臟病(chronic kidney disease, CKD)為腎臟逐漸喪失功能之疾病。全球慢性腎臟病的盛行率約為13.4%，目前全世界受CKD 1-5期影響的總人數估計為8.436億。腎功能之主要指標為腎絲球過濾率GFR，根據KDIGO指引，如有任一腎損指標持續三個月以上，應綜合考量GFR（通常由血清肌酸酐濃度、性別、年齡、種族等變數經特定計算公式估算得到eGFR值）以及尿微量白蛋白數值，以評估個案是否確診為慢性腎臟病，進而將其分期以確定適當的臨床處置方式。當個案的GFR值低於60 mL/min/1.73 m²長達3個月時，無論微白蛋白尿檢驗數值是否異常，皆認定為慢性腎臟病CKD第3a期以上，此時腎功能已受損達一定程度，建議採取醫療措施（如藥物治療、飲食和生活方式調整等），以減緩腎損惡化。如未積極治療，可能導致疾病進一步惡化，增加併發症風險。當腎損程度達末期腎衰竭時，患者需要考慮以血液透析或腹膜透析等治療方式來改善症狀與生活品質，同時延續生命。 慢性腎臟病為綜合性疾病，慢性腎臟病的診斷與病程追蹤亦非依賴單一指標。硫酸吲哚酚(Indoxyl Sulfate, IS)濃度量測可提供醫師與患者有關腎功能狀況及相關疾病風險的重要資訊，有助於傳達更全面的臨床治療與管理策略。 硫酸吲哚酚(Indoxyl Sulfate, IS)是一種親蛋白質尿毒素，由腸道中的菌群分解食物中的色胺酸(Tryptophan)產生，並在正常狀況中透過腎臟排出體外。然而，在CKD患者中，因其腎功能受損(腎絲球過濾率GFR下降)導致IS的累積，使其濃度在體內升高。這種尿毒素的累積對腎小管和腎臟組織產生毒性，包括透過誘導氧化壓力和降低抗氧化能力造成腎小管間質損傷，亦可引起腎小管細胞的凋亡和壞死，並促進纖維化相關因子的產生，進一步導致整體腎臟結構損傷和腎功能喪失，推進CKD的疾病進程，甚至進入必須接受血液透析等治療的末期腎臟病(End-stage renal disease, ESRD)階段。在CKD患者中，血清IS濃度與腎絲球過濾率呈現負相關，與死亡率則呈現正相關，IS也可能對其他器官或系統產生不良影響，包含但不限於腎性骨病變、肌少症、神經退化疾病、心血管疾病如動脈粥狀硬化等。
